Champlain College, in accordance with Vermont law, requires certain immunizations for college students.

The College requires proof of the following immunizations for all matriculated students, including part-time, unless they have a College-approved exemption (see below for details on exemption). Students who have not fulfilled these immunization requirements will be prevented from registering for the next semester¡¯s classes via a ¡®registration hold¡¯.
Records are submitted to the Patient Portal (champlainportal.pointnclick.com).
Immunization Requirements
- Tdap (tetanus, diphtheria, pertussis) vaccine | 1 dose
- MMR (measles, mumps, rubella) vaccine | 2 doses OR proof of positive antibody titers (blood test) for each measles, mumps, and rubella
- Hepatitis B vaccine | 2 or 3 doses (full series, brand dependent) OR proof of positive antibody titer (blood test)
- Varicella (chickenpox) vaccine | 2 doses OR proof of positive antibody titers (blood test) OR documentation of history of disease
- To document history of chickenpox disease, complete the Documentation of Varicella Disease form (also?found in Patient Portal “Downloadable Forms”)
- Meningococcal ACWY (quadrivalent meningococcal conjugate) vaccine | 1 dose, required for all students living on campus. Must be administered on or after their 16th birthday*
Note:?COVID-19 and influenza vaccination is strongly recommended but not?required
*Vaccination for Meningococcal B (MenB) is not required, however it is recommended as it provides individuals with the broadest coverage for current circulating strains of meningitis
Immunization Review Process
Submissions will remain listed as “not-satisfied” in your Patient Portal until Student Health Center staff have reviewed and verified your documents.
Student Health Center staff will contact you directly via Patient Portal if the documentation provided is not compliant with requirements.

Instructions For ALL Students to Complete Immunization Requirements
- Go to?champlainportal.pointnclick.com?and click ¡°足彩胜负14场 into Champlain using Google¡±
- Enter your Champlain MyMail username and password
- Enter your date of birth and click ¡°Proceed¡±
- You will see several menu options on the left side of the screen. Begin by clicking ¡°Profile¡± and confirming that all of the information is accurate
- Next, select ¡°Forms and Immunization Requirements.¡± This section includes several consent forms, your health history, and immunizations
- After you complete the consent forms and health history,?click ¡°Update¡± next to?¡°Immunization Records.¡± You are required to upload a record of your immunization history. If you do not have your complete immunization records,?you can request a copy from your healthcare provider, or?you may download and print the form we¡¯ve provided and have it completed by your provider, then upload it.
- After uploading a complete immunization record, please enter the dates you received each required vaccine by clicking ¡°Update¡± next to each vaccine name. Student Health Center staff will review your information and contact you if more information is needed for clearance.

Immunization Exemption
While religious and medical exemptions are permitted, please note that as of July 1, 2016, the State of Vermont does not allow for philosophical exemption for required vaccinations.
- Go to?champlainportal.pointnclick.com?and click ¡°足彩胜负14场 into Champlain using Google.¡±
- Enter your Champlain MyMail username and password.
- Enter your date of birth and click ¡°Proceed.¡±
- Go to the ¡°Downloadable Forms¡± section, found on the left hand side of the page.
- For general vaccine exemption: download the ¡°Immunization Exemption Request Form¡±, and complete the form based on the immunizations you wish to be exempt from, and the type of exemption you wish to pursue.
- Note that Medical Exemptions for any immunization require signature from your healthcare provider.

- Upload your complete Exemption Form under the section it was downloaded from.
- Send a Patient Portal secure message or email?healthservices@champlain.edu?to inform Student Health Center staff of your Exemption Form submission.
